To help expedite the process for your RGA request, complete the following form. Once we process your form, Candela will contact you via email to inform you that your return label has been sent via mail and should arrive shortly. Robertson places a date code label on each ballast shipped from the factory. This is a white label with a series of numbers on it (5 or 6 digits). All Robertson returns must be submitted with a valid date code for each ballast returned. If you are returning a case, one date code from one ballast is sufficient. Please record all of the numbers listed in the appropriate space below. Note: If your ballast was not purchased directly from Candela, this request is for warranty replacement only.
